There was a lot of interest in the Cardiac Rehabilitation Program referral process as a result of last week’s VCH news story. This week, we include some information about the VCH programs that are available.
The referral form that was highlighted in last week’s story was implemented July 17, 2012. It was developed to be used in VCH/PHC in-patient units, in particular, in areas that discharge patients with a cardiac related diagnosis. The referral form can be ordered through VCH printing, form number VCH.0215.
Cardiac Rehabilitation Programs in Vancouver Coastal Health include the following:
