MedRec chart tabs coming to VGH & UBCH

Do you ever have trouble finding the “Medication Reconciliation Orders” in the patient chart?

Physicians, how about discharge? Where do you readily find information about medications the patient was taking prior to admission?

“Medication Reconciliation” (MedRec) chart tabs are being phased into all charts across VGH and UBCH over the next two months. The new, clear chart tab will serve to organize the ‘Physician Orders’ section, streamlining all “Medication Reconciliation Orders (MRO)” forms and OR “Peri-Operative Medication Reconciliation Orders (Peri-Op MRO)” forms into a dedicated space in the chart, making them easier to locate. They are filed in the same, reverse chronological order process.

With the MedRec orders located in a dedicated section of the patient’s chart, important information regarding medications prior to admission is readily available for reference when writing transfer or discharge medication orders.
