Vancouver Coastal Health’s (VCH) commitment to patient safety and quality care is featured in the latest issue of Accreditation Canada’s Qmentum Quarterly journal. “Implementing Medication Reconciliation in a Large Organization” highlights leadership and collaboration as key components that enabled VCH and Providence Health Care (PHC) teams to successfully implement medical reconciliation (MedRec) on admission across the two organizations in acute and residential care.
A Required Organizational Practice (ROP) since 2006, MedRec reduces preventable adverse drug events and improves the safety and care of patients and residents. VCH and PHC have focused efforts on MedRec at admission to ensure a solid foundation for MedRec at all transitions of care.
