For every person who fills out the survey and enters your code you will receive a ballot into a draw for gift cards to Best Buy, London Drugs and Save-On-foods (value $25-$250 odds 1 in 200). BONUS – for every male you refer you will get 2 ballots! Health authority staff are eligible for all prizes.
