Accreditation Canada surveyors will be here November 26 to 29 – and we’re ready! Take our work in the area of fall prevention; it’s just one of many examples of progress we’ve made for our patients and for accreditation.
Preventing falls in acute care
The Fall Risk Screen has been added to the Patient Admission Assessment form.
A working group spearheaded by Mary Nieforth, director, cardiac sciences and special projects; Tanya Dunne, regional program lead for regional fall and injury prevention; and Prab Gill, practice initiatives lead, has initiated implementation of a two-phase regional fall prevention strategy.
Phase 1 included implementation of the Fall Risk Screen which asks if a patient has cognitive, vision or mobility impairments, altered elimination or a recent history of falls. The Fall Risk Screen has been added to the Patient Admission Assessment Form.
Phase 2 will include implementation of individualized care planning with the focus on fall prevention. This work will start in December 2012.
