VIDEO: Home is where the heart is – Part 2

Vancouver’s All Staff Forum is less than a week away but there is still time to reserve your seat at the Paetzold Auditorium or any one of the five satellite sites.

“What does Home is Best mean to you?” is the question we’ll be exploring on Wednesday, September 16 (2:30 – 4:00 PM) as we delve deeper into what Home is Best means  for our frail seniors with complex care needs  who are living in poverty and have little to no supports in the home.

For a sneak peak of what’s to come, click on the video below where Anne, Evergreen CHC Case Manager, and David, her client, talk about how we support our clients to remain independent at home with supports.

Based on your feedback from last session, much of this forum is devoted to breakout groups where you will have the chance to explore potential solutions to meet the unique needs of this client group. In order to allow for broader discussion, the forum has been extended to one-and-a-half hours.

As much of the forum is focused on discussion groups, we encourage all VCH Vancouver staff, physicians, LMC colleagues and other partners in care working in Vancouver to attend in person at either Paetzold or one of the satellite sites in order to participate fully. This will be a Forum you won’t want to miss, so register here and reserve your spot today!
