Moving house? Don’t forget to change your mailing address!

Ensuring that you have an up-to-date mailing address in your employee file is the only way to guarantee that you receive confidential work-related documents sent by mail, such as your T-4 slip, pension statements, and benefit notifications. If you have moved, and have not updated your mailing address with Vancouver Coastal Health, please do so by printing, completing and signing the address change form at the following link:

The completed the form should be returned to Employee Records & Benefits using either of the following methods:

 Scan and email to: benefits@hssbc.ca, or fax to: 604-297-9316

 This one easy step will ensure we have your correct address on file!
