Based on current research, we know that as long as the right supports are in place, a person’s home is the best place for them to recuperate from illness, manage chronic conditions or live out their final days. As highlighted in a recent bulletin for Home and Community Care staff, we call this approach to care and service delivery “Home is Best.”
A number of initiatives in each Community of Care fall under the Home is Best umbrella. Some projects you may have heard of include Home First, AURAA, and Ideal Transitions Home.
These initiatives encourage us to be more flexible and look for ways to safely support and provide care for people in their homes. It’s about improving the quality of life for our clients.
As one client’s daughter explained: “If it had not been for Home First, my mother would not be here today. She was meant to go to the extended care facility and would have probably passed away by now. But instead, I had lunch with her on the deck at a restaurant in Kerrisdale.”
