Advance Care Planning is a way for capable adults to discuss and document their wishes about the health care they would want to receive in the event they could not speak for themselves. Health care professionals have an important role to play in talking to their patients about Advance Care Planning, making the rest of the team aware of any planning the patient has done, and working with substitute decision makers to ensure that patients’ wishes are respected. The ACP Team has worked with programs across VCH since December to provide education and implementation support, and to help incorporate ACP into practice. Thank you to all programs who have been making progress on this important element of patient centered care.
