New MedRec order forms for acute care
Updated Excelleris PharmaNet Medication Reconciliation (MedRec) Order forms are coming to acute care Tuesday, February 25, 2014.
Over the past year the VCH-PHC team leads for MedRec collected feedback from physicians, nurses, nursing unit assistants/coordinators and pharmacists utilizing the Excelleris PharmaNet populated MedRec order form for acute. Face to face meetings were held and an online survey was conducted in both VCH and PHC. As a result several significant changes have been made to the Excelleris PharmaNet MedRec Order forms. The revised forms still contain all the necessary information yet appear less crowded and are more user friendly with improved flow across the form. It is expected these will be last revisions as we await the arrival of Cerner. For more information on the new forms contact your local MedRec lead(s).
As always MedRec on admission is all about the process – interviewing the patient, family and/or care provider to obtain the Best Possible Medication History (BPMH) and writing admission orders while resolving any identified discrepancies.
