CST Update for Week of May 12

Clinical & Systems Transformation is in the design period of the Design, Build and Integrate phase of the project (Phase two). This phase is expected to take a total of 18 months and is a key period which will inform the final processes and system that will rollout to the three Health Organizations (VCH, PHSA and PHC).

Implementation of the new clinical information system and processes throughout the Health Organizations will begin in the fall of 2015. We’ve included a roadmap of the rollout on CSTproject.ca and we’re updating our site on a regular basis. Design – Taking a look at clinical documentation and emergency design work
The clinical design teams are in week 6 of the 26-week design phase. We’ve highlighted the work of two teams this week; Clinical Documentation and Emergency.

To date, the Clinical Documentation team covered topics such as start/change of shift, staff assignment, vital signs, height/weight admission assessment, shift/ongoing assessment, problem list/diagnosis list, isolation/precautions, and allergies. Last week was also the kick off of the team’s Maternity Clinical Documentation stream. The Maternity design will run for six weeks and will cover topics such as: OB registration, labour and delivery, newborn assessment, emergent OB process, post partum care, and Newborn & NICU transfer.

The Emergency team designed standardized Adult and Pediatric Triage forms through exhilarating conversations on transforming practice for best patient care. The team is comprised of a very engaged interdisciplinary team, who provide valuable input on all aspects of patient care to improve the patient and provider healthcare experience.
