The My Health My Community survey officially closes in just two weeks! If you haven’t completed the survey yet, there’s still time to do so and be entered to win some great prizes such as an iPad. Take the survey on the My Health My Community site.

If you completed the survey – thank you! There’s a special contest for people who have done the survey. For every person who fills out the survey and enters your code you will receive a ballot into a draw for gift cards to Best Buy, London Drugs and Save-On-foods (value $25-$250, odds are 1 in 200). BONUS – for every male you refer you will get two ballots! Health authority staff are eligible for all prizes.

We still need more surveys completed
We’ve come a long way since we started in June 2013 and we are so close to our goal! But we still need about 1,400 people in the VCH region to complete the survey so that the results are statistically meaningful.
