eCommunityNEXT: Working together for better care
In February 2015 the eCommunityNEXT Project began a two-year journey to undertake a Community Clinical and Systems Transformation (CST) to standardize and streamline workflow within and across programs, improve community analytics and management reporting, and address the current user and technical limitations of PARIS.
Since then, the project has been actively engaged with directors, managers and subject-matter experts across the region to undertake the work needed within four key areas: Care planning, to help standardize and streamline workflow, operational reports, and care planning processes within and across programs; Assessments, to simplify, streamline and standardize where possible; Community Analytics and Management Reporting, to support clinical decision making and timely reporting; and a PARIS v6.0 system design, build and test, with easier navigation, a reduction in duplication of information, enhanced information sharing and improved usability to support clinical workflows.
Care planning
The project team has been activity engaged in workshops with Subject Matter Experts from across the Community programs, collaborating on the design of the new care plan and to develop standardized inter-disciplinary care planning processes to enable clinical care within and across programs. In essence, the care plan provides clinicians with a snapshot view of “Who is involved?” and “What’s going on?”
The first version of the care plan for PARIS is now being demonstrated to programs across the organization, with very positive feedback:

Assessments
The project team is working with Subject Matter Experts from across the programs to gather requirements and determine the needs of each program as we begin the work to simplify, standardize and streamline our assessments. The first assessments out of the gate are the MH&A Initial Assessment, Home Health Initial Assessment Tool, and for Public Health, the Maternal/Newborn Assessment.

Community analytics & reporting
A key area of focus for the project is to improve our Community analytics capabilities, which includes the development and implementation of new reports for all Community programs. This work involves the migration and updating of existing reports as identified and validated through our work with our Community Analytics & Reporting Subject Matter Experts. The first new analytical tool to be developed is the PARIS Volume Cube.

What is the PARIS Volume Cube?
The PARIS Volume Cube is a flexible, intuitive tool that allows those who use management reports to group and view data in ways that were not possible before. The cube contains new, active and discharged referral volumes by time, region and program, and provides referral reason, discharge disposition and demographics information. What previously would have required multiple reports to be run separately can now be shown in one single screen with minimal loading time. The cube will be expanded to include more measures, fields, and features in upcoming versions.

PARIS v6.0
The PARIS version 6.0 upgrade will enhance navigation and usability to support best practice workflows, and allow the easy upload of external documents to the system.
An improved design and navigation will help care providers find what they are looking for more easily. Staff will be able to share information with other care providers in a more efficient and effective manner.

In summary
Streamlining and standardizing how we provide care across all Community portfolios and implementing a significant upgrade to PARIS, will provide a greater level of accurate, consistent, real-time information in one sharable client record. Clinicians will have better tools to support point-of-care decision making. The comprehensive client data we will have as a result of this work along with improved and flexible data analytics will drive program planning and care decisions toward better illness prevention and wellness promotion.
