Reminder to complete survey before May 9, 2016

Last week you were invited by email to participate in this study. If you have completed the survey, thank you!

If you have not yet completed the survey, please take 10-15 minutes to complete the survey, voice your experience of the policy and enter a draw to win a $500 Visa gift card. All health authority employees who worked during the 2015/16 influenza season are invited to participate.

Clicking on the link and completing the survey implies your consent to participate is given and you agree to the following:

  1. You have read the Invitation to Participate letter (attached) and have had an opportunity to have all your questions about the study answered to your satisfaction (email questions to HCW.FluSurvey@bccdc.ca).
  2. You understand that participation is voluntary and you can withdraw from the study at any time. You will not suffer any negative consequences, personal or professional, by doing so. Because the survey is anonymous, data cannot be withdrawn after submission.

The survey does not collect any personal identifiers, your computer’s IP address will not be collected, and all data collected will remain confidential.

Technical issues

Due to unanticipated traffic and technical issues some participants are experiencing delays when accessing the survey or are unable to complete the survey. We apologize for any inconvenience this may cause, and please be assured that your response is important to us. The survey will be open for responses until May 9, 2016.

If you experience delays or issues when completing the survey you can use the “Save and continue later” button to return to the survey any time before May 9, 2016.
