Putting clients and their families at the centre of care is what we all do every day.  It’s an automatic reflex, and sometimes we don’t even realize when we do it.  We want to highlight the many examples of client and family centered care that we know happen all the time, but are rarely celebrated.

Share your stories with us

Tell us about a time you or your team went above and beyond in partnering with clients or their families for a positive outcome. We are looking to celebrate the simple and complex stories.

One team or individual submission will be awarded a free registration to this year’s Quality Forum (http://qualityforum.ca/).  A selection of submissions will also be shared in the VCH News.
