Heart Failure (HF) is a leading cause of hospital admission. There are a number of known gaps in care for patients with HF. For example, challenges in the transition from acute care to home, often leads to high rates of Emergency Department visits and readmissions.
The VCH/PHC regional Heart Failure Strategy in conjunction with the Clinical Guidelines Initiative is working towards improving HF management in both acute and community settings by streamlining and standardizing care according to evidence-based best practice guidelines. Beginning February 4, 2013 Richmond Hospital will go live with the new Heart Failure pre-printed order package. The package includes:
- Heart Failure Adjunct Orders (VCH.RD.RH.0287) – To be used in conjunction with admitting orders or other PPO’s. Can be used at any point during the admission.
- Heart Failure Teaching Record – VCH.0124 – Multidisciplinary tool used to facilitate documentation of education related to self-management skills.
- My Daily Fluid Restriction Worksheet – VCH.0128 – Patient tool used to track own fluid intake.
- Heart Failure – Inpatient Education Package – FD.780.H432 – Given to patient with appropriate HF self-management teaching.
