Advance Care Planning (ACP) education sessions are underway in Richmond until the end of March for staff in Acute, Community and Residential care settings.
ACP is a way for capable adults to discuss and document their wishes about the health care decisions they would make in the event they cannot speak for themselves. VCH staff play a pivotal role in implementing and supporting ACP in our facilities by using the VCH ASK model. The ASK model enables staff to share in the conversation of ACP by:
- Asking patients if they have thought about future health care decisions
- Sharing and storing the information with the health care team (in a Greensleeve or in PARIS)
- Knowing your patient’s wishes and where to find ACP resources
