MedRec chart tabs coming to Richmond

Do you ever have trouble finding MedRec orders in the patient chart?

Well that will be a thing of the past, now that Medication Reconciliation chart tabs are in Richmond! The new, teal coloured chart tab will be located immediately after the ‘Physician Orders’ section and is intended to give all MedRec documents a dedicated space in the chart, making them easier to locate.

As Medication Reconciliation continues to be implemented for admissions, transfers and discharges across our community of care, patients will have Medication Reconciliation completed at all transitions and the chart tab will allow these to be kept together in one location for ease of access.

The tabs are being added to Richmond acute and residential charts during the week of June 17th.
