bestPATH set to begin implementing improvement ideas
After meeting with stakeholders, conducting workshops and undertaking “shadow shifts”, bestPATH will now begin developing and implementing some of the ideas for improvement – including a person-centred care plan.
bestPATH (Person-centred, Appropriate, Timely Healthcare) is a partnership between Richmond acute and community to develop and implement a strategy that will enhance the care and transition experience for patients and clients, improve patient access and flow through the healthcare system, and improve workflows for staff and physicians. The goal is to provide the right care, in the right place at the right time in a positive, supportive environment.
The bestPATH team spent the past few months meeting with various stakeholders, including holding workshops with patients/clients and their families, staff and physicians. The focus of the work has been to engage our stakeholders, fully understand the current state of patient care in Richmond, and identify what the future could look like.
Some frontline staff and physicians also engaged in a “shadow shift” between acute and community with the goal of better understanding each other’s work environment, challenges and opportunities. Additional details regarding their experiences will be coming out in a future VCH News article.
As a result of all of this work, a number of improvement ideas were identified and prioritized. The bestPATH steering committee, working group and frontline staff will begin developing and implementing some of these improvement ideas over the next few months. One idea in particular, a care plan developed from the patient perspective that would follow them through the hospital and into their home, was consistently agreed to by all stakeholders and seen as a tool that could support high quality patient care.
