Richmond MH&A is launching the much-anticipated evaluation of its Person & Family Centred Care Initiative.In partnership with Simon Fraser University Research Department, the on-line survey will be available at Checkbox and in print copy, for 4 weeks beginning May 16 and available until June 10, 2016. Are you a Richmond MH&A staff member? Do you know someone who is receiving care in Richmond Mental Health and Substance Use?  Would you like to help us improve the quality of Mental Health & Substance Use Services?

If you answered “yes” to any of the above, we’d like you to take our Person & Family Centred Care Patient/Client Survey, or share the survey links below with the person receiving service.

This survey takes about 10 minutes and asks about your experience with Richmond Mental Health & Substance Use, including:

  • Communication with the Care Team
  • Involvement in Care Unit Environment
  • Supports and Services
  • Discharge Outcomes
