Vancouver teams to present at 2013 Quality Forum: February 28 – March 1
Professional Practice (nursing) team members Harjender Walia (clinical educator) and Lori Campbell (clinical resource nurse) and Quality, Patient Safety and Infection Control Data Analyst Jeffrey Reimer, will be presenting at the upcoming Quality Forum hosted by the BC Patient Safety & Quality Council.
Harjender and Lori will be discussing the Vancouver Acute medication reconciliation implementation in a presentation called “Medication Reconciliation – Vancouver Acute Improves Patient Safety”. The session will include highlights such as “Why Reconcile?” and explaining that “over half of medication errors occur at the interfaces of care” (Rozich JD, Reser RK. Medication Safety: One Organization’s Approach to the Challenge. J Clin Outcomes Manage. 2001;8(10): 27-34). They will share the acute care MedRec process including planning, education, implementation, and sustainment strategies.
Jeffrey Reimer, will be presenting “Optimizing Medication Reconciliation Data Submission and Reporting for Residential Care.” His presentation will highlight the creation of an efficient Excel and SQL server based data submission method that allows residential care facilities to report MedRec completion in accordance to performance measure requirements mandated by the MoH for Clinical Care Management, VCH-PHC Clinical Care Guideline Initiatives and Accreditation Canada.
