Stabilization care for complex clients now open at the Dr. Peter Residence
Vancouver Coastal Health and the Dr. Peter Centre (DPC) have collaborated to transition six residential care beds to stabilization care beds for complex clients at the Dr. Peter Residence. Care is provided for up to three months to clients who are experiencing significant health and social challenges including substance use, mental health issues and physical health concerns. DPC provides client-specific care to assist clients to stabilize their health and overall functioning so they can return to the community with the necessary supports.
Referral Information:
Clients who are HIV positive have priority for available beds; however, non-HIV clients will be accepted if an HIV positive client is not waiting. For community-based clients, referrals can be made by the Most Responsible Clinician (MRC) to VCH Priority Access using referral forms available online.
For clients in hospital, referrals are made via TST. Community providers remain engaged with the client and DPC during the client’s stay at the Dr. Peter Residence, through regular case conferences and the development of discharge plans to support successful reintegration in the community.
