Congratulations go out to the Vancouver Coastal and Providence Healthcare residential care MedRec teams who achieved a combined performance level of 98% for successfully completing MedRec on admission and/or re admission! This accomplishment is well above the defined target of 75% and demonstrates the residential care teams’ dedication and commitment to safe and quality care for our residents.
The 21 VCH-PHC residential care teams’ success has been supported through collaboration with VCH the Quality and Patient Safety (Leslie Forrester and Jeff Reimer). As a result, a completely automated reporting and submission process for MedRec residential care data has been developed. Working together has created success for all.
