New policy: Preparation of Parenteral Medications for Administration by Syringe

The practice of pre-drawing medications into syringes (at any time other than immediately prior to administration) presents opportunities for contamination and error.

The purpose of this policy is to provide direction regarding the practice of preparing syringes with parenteral medications including information on preparation, labeling and disposal.

This policy applies to acute, residential and ambulatory care settings (please refer to the posted policy for additional details).

The policy has undergone extensive interdisciplinary stakeholder consultation and will also be reviewed in 6 months at local Medication Safety Committees for feedback.
