Aboriginal patient navigators: Bridging the cultural gap

The Aboriginal Patient Navigator Program helps Aboriginal people access health services.  A navigator acts as a bridge APN postcardbetween the patient, the community and the health care system.

APNs offer consultation and support services to any First Nation, Métis or urban Aboriginal person within our region and also take referrals from all health authorities for patients traveling into Vancouver for medical services. The APN team also supports heath providers, assisting in all areas of patient and case planning, bridging the cultural gap, and by connecting providers to community resources either Aboriginal based or culturally specific. APNs contribute greatly to patient safety and help raise awareness of the needs of Aboriginal patients throughout our system. VCH recognizes the APNs for their valued contribution in our system and for helping patients navigate for the safest possible patient journey.

May 22-25, 2013 Canada celebrates Aboriginal Awareness Week (one of the designated health promotion weeks for Health Canada).
