A new incident investigation tool: coming June 18th!

Workplace Health has collaborated with all BC health authorities incident investigation tool
to develop a new quality incident investigation tool to support
managers who are responsible for ensuring investigations are completed for staff who have been involved in work-related incidents, injuries, illness or near misses. Currently, if staff have a work-related incident, they contact the Workplace Health Call Centre to report the incident. The call centre agent records the information and sends a request for investigation to the appropriate manager, which includes a fillable-pdf with blank sections at the end to record contributing factors and corrective actions.

Starting on June 18th, managers across BC will instead receive an Excel file from the call centre agent. After enabling macros in that file, and after assembling the Investigation Team, the incident details are confirmed and the investigation is guided by a series of themed questions. As the questions appear and are answered, contributing factors are suggested. Pilot-testing with subject matter experts and selected managers across the province has confirmed that the new tool will help produce quality incident investigation in a timely manner.

There are additional resources that will available to support the new tool:

  • An e-learning course on CCRS that teaches you how to conduct an Investigation from start to end. Search for “Incident Investigations” in CCRS under “course search”: https://ccrs.vch.ca
  • A video tutorial on the Workplace Health intranet page that shows you how to complete the tool itself
  • A Field Guide that can be printed out and used by the Investigation Team at the scene of the incident to gather all the required information.
  • A blank version of the electronic tool that can be filled out prior to receiving the email notification. This “Pre-Workplace Health Call Centre Notification Incident Investigation Tool” (aka PNIIT) can be used to document the investigation as soon as the incident is reported.
