The new Incident Investigation Tool for managers is live!

Incident investigation toolIn our ongoing commitment to providing a safe workplace for our staff, Workplace Health has implemented a new Incident Investigation Tool to help managers produce quality incident investigation in a timely manner. The new tool has replaced the current ‘fillable-pdf form’ with blank sections at the end which Managers currently use to record contributing factors and corrective actions for work-related incidents, injuries, illness or near misses.

The new quality Incident Investigation Tool:

  • Still requires participation from the Investigation Team and employee, who is still required to call the Workplace Health Call Centre ( 1 866 922 9464) to report a work-related incident
  • Will be sent to the Manager by the call centre agent for completion in a new excel format  
  • Requires ‘macros’ to be enabled in the file prior to recording the incident details
  • Helps managers collect all relevant incident details by guiding them through a series of themed questions
  • Will suggest ‘contributing factors’ as the manager completes the questions

Work-related incidents that have to be reported to Workplace Health include: injury at work, exposure to blood, body fluids or other chemicals, near-miss incidents or if you have suffered psychological trauma or acts of violence/aggression.

Additional resources are available to support the new tool:

  • An e-learning course on CCRS that teaches you how to conduct an Investigation from start to end. Search for “Incident Investigations” in CCRS under “course search”: https://ccrs.vch.ca
  • A video tutorial on the Workplace Health intranet page that shows you how to complete the tool itself
  • A Field Guide that can be printed out and used by the Investigation Team at the scene of the incident to gather all the required information.
  • A blank version of the electronic tool that can be filled out prior to receiving the email notification. This “Pre-Workplace Health Call Centre Notification Incident Investigation Tool” (aka PNIIT) can be used to document the investigation as soon as the incident is reported.
