You can now submit your requests for photo identification through an e-form available on FH Pulse.

Who can use the link?

Any employee with a health organization login (Fraser Health, Vancouver Coastal Health, Providence Health Care, Provincial Health Services Authority).

How do I access the link?

Via a health organization computer.

What internet browser is needed?

Internet Explorer 7 or Chrome.

Any special notes?

Chrome users need to tab through the fields, the mouse only works on the ‘last name’ field to reset.

What happens after I fill in the eform?

• Once you fill in the e-form you will receive a confirmation e-mail that your request has been received.
• Once your request has been processed, you will receive an e-mail advising that your photo identification has been printed and will be sent out to the address provided with the request.

Facts about your Photo ID:

Access:

• You only require one Photo Identification card to access all sites, from Hope to Sea to Sky (includes all facilities within the four health organizations that are part of our access control systems). Multiple health organization access can be coded on to one photo identification card.
• If you require extra access, fill in the e-form (http://fhworkspaces/sites/PhotoIDAccessRequest/Pages/Form.aspx), including your Manager’s e-mail address for approval.

When do I need a new photo?

1. If this is your first photo identification badge with the four health organizations.
2. If your photo is greater than five years old and you require a replacement badge.

Can I send you a ‘selfie’?

• No, we only accept photos via site Security and authorized personnel.
