UBCH MH Team: Keep up the fabulous work on the interprofessional comprehensive care plans!

VGH and UBCH celebrate the final phase of 48/6!

Over the spring and summer, the Preadmission Clinic, Surgical and Cardiac units at VGH, as well as the Inpatient Psychiatry units at VGH and UBCH went live with 48/6.

The more we know how our patients were functioning prior to their acute care admission, the better we are able to support them to return to their previous level of function.  The core elements of 48/6 are comprehensive admission screening, and personalized care plans. These are now part of the care team’s standard work.

As Carrie Willekes, Head Nurse from Palliative Care states; “It is just good nursing – instead of having the information in our heads, we write it down so everybody knows”.

In a recent walkabout, Sue Goulding, VCH Project Manager and Selina Wong, Vancouver Acute Project Lead heard positive feedback from various point-of-care staff who have embraced it, such as:

“It is fantastic! It prevents patients from falling through the cracks. We are all on the same page and it doesn’t take much effort” says Annie, RN, VGH Spinal Unit.

“I really like 48/6… especially when I have a new patient. I know exactly what I need to do to look after my patient from the care plan” another RN from the Neurosciences Unit commented.

Congratulations to ALL of the units/programs across Richmond, Coastal and Vancouver Acute who have worked hard over the last three years to incorporate the 48/6 core elements! Special kudos to sites/units who demonstrated exemplary results during recent chart reviews:  Bella Coola, Bella Bella, VGH Palliative Care team, Inpatient Psychiatry at UBCH and VGH, Preadmission Clinic and various surgical units.
