The new PARIS clinical care plan now live

The eCommunityNEXT Project is excited to announce that on November 7 the NEW PARIS Clinical Care Plan went live in Read Only for all PARIS Users, and Wave 1 teams are beginning to document!

The new care plan supports an interdisciplinary cross-program approach to client care, allowing clinicians to see who is involved and what is going on in the care of their client.  It is a standard tool in a single location for sharing a client’s Needs, Goals and Interventions, reflecting the partnership between clients, informal supports and clinicians.

Access to document within the new care plan will occur as the project team implements in three waves over the coming months.  Wave 1 began on November 7, which includes teams from:

  • Richmond Home Health, Mental Health & Substance Use and Public Health
  • Powell River Home Health, Mental Health & Substance Use and Public Health
  • Vancouver Mental Health Northeast
  • Vancouver Evergreen Home Health and Addictions

eCommunityNEXT Project team members are now on the ground working with Wave 1 Coaches and Leads, providing additional support as their teams participate in training and begin to document in the new care plan.  In just three days, feedback across the programs has been very positive with many clinicians recognizing how it supports integrated client care:

“This care plan will support further integration with our Mental Health Services.” (Mental Health & Addictions)

 “I see the value in the new care plan as a snapshot of client care across programs.” (Home Health)

 “I’m a care plan believer!” (Public Health)

Waves 2 and 3 will be implemented in early 2017.  Going live in waves will allow the project team to assess the overall implementation and education approach and make adjustments where needed for the following two waves.

For PARIS users with Read Only access, the project team has developed a brief video about a fictional client who accesses services across Home Health, Public Health and Mental Health and Addictions.  It shows how the clinicians involved in her care can access the new care plan to find out more about the care and services she receives. The video is available from the eCommunityNEXT Project website  and is open to anyone interested in learning more.
