Effective February 5, 2013, North Shore and Lions Gate Hospital Staff, physicians and volunteers will notice changes to the process for getting new or replacement photo ID cards.
Photo ID services is moving
Staff, physicians and all others requiring access changes or new or replacement photo ID cards will need to go to the Paladin Security Office (Room 1.1.17 in the LGH 15th St. Lobby near the LGH Foundation office). Security staff will be available to take ID photos during the following times:
- Tuesdays – 0800 to 1200 hrs
- Thursdays – 1200 to 1600 hrs
- Any day of the week – 2100 to 0700 hrs
- New Employee Orientation days (monthly) – 1100 to 1300
We encourage individuals to book an appointment as they will be served before those without a booking. Times can be scheduled by emailing the Security Supervisor at lg@paladinsecurity.com or by calling 604-984-5943.
Access cards to take a few days
Individuals requesting new or replacement cards will arrive at the Paladin Security Office within five business days of being requested and can be picked up at the times listed above.
Alternatively individuals can have their photo taken and ID produced immediately by travelling to the centralized Photo ID Processing Centre at Vancouver General Hospital. Advance appointments are required and can be made by calling 604-875-4111 ext.20003 or emailing lghphotoid@vch.ca The office is located in Room 179 of the Willow Chest Centre, and hours of operation are:
Monday – Friday, 0800-1600 hrs (closed 1200-1300 hrs).
Departments where staff or physicians require immediate card access to perform their job functions can request a small supply of preprogrammed blank access cards. These cards are intended to be provided to the staff or physicians until their own photo ID cards are ready for pick up.
