Advance Care Planning: Ask, Share, Know

Advance Care Planning (ACP) is one way that we at VCH-Coastal are striving to meet our True North goals of ‘people first’ and ‘providing the best care.’

Advance Care Planning is a way for capable adults to discuss and document their wishes about the health care they want to receive in the event they cannot speak for themselves.

Coastal staff play a pivotal role in implementing ACP in our facilities by using the VCH ASK model. The ASK model enables staff to share in the conversation of ACP by:

  • Asking patients if they have thought about future health care decisions
  • Sharing and storing the information with the health care team
  • Knowing your patient’s wishes and where to find ACP resources
