Good Catches show initiative and persistence

Congratulations and a big thank you to these four talented staff who noticed something amiss and took steps to correct it. They were recognized at the recent Staff Forums for their high level of commitment to patient safety.

Davender Aujla, LPN, Minoru Residence – Davender found an incorrectly labelled medication in the wrong medication pouch. She notified Pharmacy right away, so the label was changed and medication dose placed in the correct pouch.

Jane de Lemos, Clinical Pharmacy Specialist, Richmond Hospital Emergency Department – After catching an error around insulin administration, Jane took the initiative to review the last three years of insulin incidents reported in SLS for Richmond Hospital. She noticed a trend, identified the cause and recommended changes to a line entry to better help nurses identify the correct vial.

Carrie Mowat, LPN, Minoru Residence – Carrie noted a medication was unavailable for a 2100 dose. After double checking with Pharmacy, she discovered that the medication had been discontinued two days earlier and the resident no longer required it. In another instance, when checking a resident’s medication profile for a medication to be given once daily, she found there was no doctor’s order for it. This was brought to the attention of Pharmacy and was corrected.
