Do you know where to obtain drug information quickly and easily?

The latest in our ongoing series of VA Medication Safety Committee bulletins tells you:

  • where you can get pocket references of various medication guidelines
  • how to access the Formulary
  • how to locate online resources on VCH Connect, and
  • how to  access a pharmacist 24 hours a day.

In the light of Vancouver Community of Care Accreditation 2012, we’re taking a closer look at our everyday processes and ongoing quality improvement in how we work with and for our patients.

The other bulletin posted at the same time discusses reporting medication incidents.

Reporting medication errors or “near misses” helps everyone to improve their medication safety.  Staff members are encouraged to file detailed reports on errors or close calls on the Safety Learning System on VCH Connect; the bulletin shows you exactly where to find it online.

Your reports may lead to improvements in procedures that reduce or prevent future errors. Also, trends can be spotted over time and system changes can be made to counter them.
